Pulse Oximetry Explained: Why Nurses Monitor SpO2 So Closely

Oxygen delivery is a fundamental process to every system in the human body. Our cells rely on it to function and replicate, and impaired gas exchange can escalate quickly into serious circulation complications affecting the brain, heart, and other organs.

Pulse oximetry gives nursing staff a continuous, non-invasive look into a patient’s oxygenation status. It’s one of the most frequently checked vital signs, and often the first objective signal that something is starting to go wrong.

Additional signs of hypoxia

Pulse oximetry is a small clip-on or wrap-around device that measures blood oxygenation (SpO2) and heart rate. The patient's oxygen saturation level is presented to the nurse as a percentage, indicating the amount of oxygenated hemoglobin perfusing the peripheral tissues.

When performing this intervention, the nurse must select a site with good perfusion, indicated by an adequate capillary refill response. Fingers often work well for adults and children, whereas the wrist or foot is preferred for infants and newborns.

Anatomy and Physiology Refresher:

One red blood cell contains millions of hemoglobin molecules. Each hemoglobin molecule contains four heme groups and are capable of binding one oxygen molecule. If something is preventing an oxygen molecule from binding, less oxygenated blood is delivered to the tissues.

Hypoxemia (reduced oxygenated blood) → Hypoxia (reduced oxygenated tissues)

Normal SpO2 readings on room air are between 95–100%. Nurses must be thorough in their patient assessment, as target numbers vary based on patient health status and comorbidities (e.g., some patients with chronic respiratory conditions have a lower individualized baseline).

But when those percentages trend downward, nurses begin to suspect a developing issue.

Routine vital sign monitoring is an important part of patient safety and stability in the clinic. Checking SpO2, alongside temperature, heart rate, respiratory rate, and blood pressure, is a standard practice. Frequency of monitoring is guided by unit policy and patient acuity.

Nurses are on the lookout for abnormal findings and trends. A gradual SpO2 drop from 99% to 91% over the span of a few hours is clinically significant. While numbers can fluctuate for benign reasons, including anxiety, motion/movement, or a shifted or obstructed nasal cannula, a sustained drop in oxygen is a sign of respiratory deterioration and warrants further assessment.

Common clinical presentations that call for frequent SpO2 checks:

  • Post-operative recovery after general anesthesia

  • Chronic respiratory conditions (cystic fibrosis, COPD, asthma, etc.)

  • Cardiac conditions with pulmonary involvement (heart failure, pulmonary embolism, etc.)

  • Procedural sedation

  • Patients receiving supplemental oxygen therapy

Why do nurses check pulse oxygen so frequently?

Before escalating an abnormal reading, the nurse reassesses. That means troubleshooting first: confirming the oxygen delivery device (nasal cannula, non-rebreather mask) hasn't shifted or become obstructed, repositioning the patient to optimize airflow, and verifying the equipment is properly operating. If the drop holds up, the nurse notifies the charge nurse and physician.

Relevant details to communicate include the reading itself, the trend (baseline reading and time, compared to the current reading), the oxygen delivery method (room air vs. supplemental oxygen, and flow rate), and how the patient presents (retractions/work of breathing, color, mental status, and vital sign trends).

The nurse can recognize and analyze cues that the patient is experiencing respiratory distress and hypoxia:

Acute signs:

  • Cyanosis (bluish tint to nailbeds, lips, oral mucosa)

  • Increased respiratory rate (tachypnea)

  • Shortness of breath (dyspnea)

  • Rapid heart rate (tachycardia)

  • New confusion or altered mental status

Chronic sign:

  • Finger clubbing (develops over time with long-standing hypoxia; fingertips bulge and nails curve down

How abnormal oxygen saturation is reported

Pulse oximetry is a simple instrument on the surface—a clip or wrap, a percentage, a flash and beep on the monitor. But the way nurses use it reveals what great clinical monitoring design has to account for: the SpO2 trend.

A gradual drift, a sudden drop, and a mismatch between the reading and how a patient looks all trigger clinical judgment.
